It is a fillable pdf, so you can type to fill in the blanks (the blue shaded portions).
The form will accept an electronic signature. You can sign electronically by creating a new digital signature or using your existing digital signature.
Save a copy of the filled-out form by saving the document (file-save as) or printing the document (control p-choosing adobe pdf as the printer) as a PDF.
Enter a new filename for your filled-out form and save it to your PC.
You can submit to the form to Shamrock by email or postal mail.
For email, send a message to firstname.lastname@example.org and attach the filled out form.
If you use postal mail, print and sign the completed form addressed to:
